Clinical observationa of 206 Cases of Blunt Abdominal Trauma


Abstract
This I-eport is consist of a review of 206 patients who sustained blunt trauma on the abdomen during past 12 years, from 1963 to 1974 and who were treated at the surgical service of Capital Armed Forces General Hospital.
The causes of bnut adominal trauma were car accidents(95cases, 46. 1%), blow and kick(69 cases, 33.5%), and fall(35 cases, 17%).
The clinical manifestation was adominal pain (90.3%), tenderness 85.9%), rebound tender
ness (74.3%), mucular rigidity(54. 9%), abdominal distension(49%), and diminished peristalsis (42.2%) and etc.
Isolated injuries were not common, and- most injured organ was, multiple ar associated with
the other organs in the intraperitoneal and¢¥ or extraperitoneal region.
The commcn organs injured were small bowel(92 cases, 35.1%), spleen(41¢¥cases, 15.7%), liver(28 cases, 10.7%), and kidney(21 cases 8%).
Abdoninal paracentesis was performed in cases, and 91 cases revealed positive finding. Among 112 cases of hollow viscus rupture, the number of positive subphrenic air shadow on plain abdominal x-ray film was 62 cases.
Simple closure was possible in minutes to the hollow viscus, and splenectomy and nephrectomy were particularly performed in injury of spleen and kidney.
The common postoperative complications were wound infection(35 cases, 41.2%), pulmonary complication(10 cases, 11.8%) and fecal fistulas(8 cases, 9.4%).
The overall mortality was 19 cases(9.2%). and the causes of death were irreversible shock(7 cases 36. 8%), septicemia(6 cases, 31. 6), and renal shutdown(3 cases, 15.7%).
